KINNECT’s tertiary qualified clinicians are experienced in the delivery of Activities of Daily Living and Home Assessments which are used to determine the medical, psychosocial and rehabilitation needs of an injured person at their home. The goal of the Home Assessment is to provide recommendations that will facilitate a return to Activities of Daily Living (ADL), following injury or illness.

Supporting a person with their ADL can enable them to adhere to their known capabilities and restrictions during the initial recovery period. Better management of the injury at home enables a speedier recovery and return to work.

Often used in complex cases, the ADL or Home Assessment analyses an individual’s functional performance in daily activities around the home.

This includes areas such as:

  • personal and self-care,
  • domestic activities,
  • general home maintenance activities.

Using knowledge of the worker’s medical diagnosis and understanding of injury pathology and recovery, KINNECT Consultants will evaluate the suitability of tasks around the home and determine any adaptions that may be required to help the individual to regain their independence.

The ADL or Home Assessment is documented in a formal report. This includes:

  • an objective report with the individual’s functional capacity and recommendations for a return to ADLs,
  • recommendations for any equipment or services that may be required to support the individual in their home,
  • a detailed plan to implement the recommendations,
  • recommendations to facilitate a safe and durable outcome.

How it Works

KINNECT is a client centred workplace rehabilitation provider concerned with promoting the health and well being of individuals. The purpose of the Home Assessment is to enable people with an injury or illness to participate in every day activities around the home to help support their recovery and return to normal life. Our focus is to support an individuals strengths and minimise the impact of their weaknesses.

What’s Included

  • An experienced Occupational Therapist and Injury Management Specialist who will visit the individual’s home to conduct the Home Assessment.
  • Identification of those duties/tasks around the home that can be safely performed by the injured person and any limitations.
  • Advise on modifications or adaptions to tasks around the home to improve safety and independence, plus any equipment that may be required.
  • Development of a comprehensive report detailing the home assessment, the workers current functional capacity for tasks around the home, any modifications, services or equipment that is needed.
  • Provide tailored education to the individual on how to do specific tasks in different ways to better manage their injury, improve safety and manage fatigue.
  • Upon request, the consultant can assist with service coordination including monitoring and organising reduction in recommended services as the individuals progresses through their recovery.

Equipment Prescription

KINNECT Injury Management Consultants are experienced at providing tailor-made advice and education to injured workers. They can explain the appropriate recommendations and individualised adaptations that can be made in the home, demonstrate alternative ways of doing particular tasks and test different methods. The goal is to improve the individual’s confidence and ability to perform tasks safety around the home.

It’s important to understand that not all adaptations or equipment that may be recommended are “major”. Often, minor adaptations such as using a shower chair or adapted cutlery can make a positive difference to the individual’s comfort and ability to do things independently.

Time

  • 2 hour Home Assessment and development of Home Assessment Report,
  • Reports are completed post assessment and submitted to you usually within 24 Hrs, though at times this can be longer if additional quotes for equipments or services are required.

Please note KINNECT provide Home and ADL Assessments for workers compensation schemes such as WorkSafe Victoria, WorkCover QLD, Comcare, SIRA, Workcover Tasmania and WorkCover WA. These assessments are set at different rates.

FAQ

When is a home or ADL assessment required?

A home assessment or ADL Assessment may be required when:

  • An injured/ill worker is returning home from hospital and the suitability of the home and the individual’s functional capacity to undertake tasks of daily living need to be ascertained;
  • It is a complex or prolonged case or where the injured/ill worker has multiple injuries that they are recovering from.
  • Advice is required regarding tasks modification, or provision of aids and appliances to help them with the Activities of Daily Living.
  • Objective recommendations about the individuals’ functional capacity are required to help inform a gradual return to work.

What is detailed in a KINNECT Home or ADL Assessment?

A Home Assessment provides information on the following:

  • An injured workers specific functional capacity for daily tasks around the home;
  • An injured workers restrictions/limitations;
  • Tasks that can be performed safely without risk of injury and/or re-aggravation;
  • A detailed plan to implement the recommendations;
  • Recommendations regarding best practice biomechanics and proactive self – management techniques.
  • Recommendations on any equipment or modifications needed around the home to improve independence and safety.

What details will I need to make a referral?

When making your referral, you’ll need the following details: 

  • Your details and your company’s details 
  • The Worker’s details including their Date of Birth 
  • Details of the Insurer and Claim Number (if the worker has an accepted workers compensation claim) 
  • Details about who should receive the invoice (this could be the Insurer or your Company if you are self-funded) 
  • Details about the injury or illness including any medical certificates or reports you have 
  • Details about the service you need (if you know this information already or are familiar with Workplace Rehab service, but don’t worry if you don’t) 
  • Details about the worker’s role including a position description, job dictionary or job task analysis (if you have one) 
  • Plus, anything else you think we need to know about the situation 

What happens next?

Once you’ve submitted the form, the details will be picked up by the Workplace Rehabilitation lead that is matched with your location. You’ll hear from us within 24 hours to confirm we have your referral and outline the next steps. 

When will the service be scheduled?

We aim to schedule your service as quickly as possible after we get your referral, normally within 48 hours, however this can sometimes be delayed if we are particularly busy in your area. We’ll keep you up to date if we think there is going to be a delay.  

Paying for the service

For most of our clients, their insurer will foot the bill for any services so you won’t need to worry, but if you are self-funded, you might want some more information about how this works. You won’t need to pay at the point of the referral. Once we know what services you’ll need, we will let you know the costs and then you’ll have a choice to pay via Credit Card or we can invoice you instead.
